For more than a decade, UniHealth Foundation has invested in programs that support people experiencing homelessness as they progress from hospital discharge to permanent housing. A wide variety of services and supports are required along the way, and UniHealth has identified many interventions that help make patients’ journey healthy and successful. In addition, Pathways To Health™ provides a variety of programs and services offered both in-house and on an outreach basis.
The program will add 15 additional navigators and community health workers to respond to referrals throughout Sacramento. However, Pathways is different than traditional homeless navigator programs. Outreach workers will provide ongoing support to enrollees through a team-based approach, working with a case manager, clinicians, and a housing specialist to support enrollees on their path to health and housing stability. These staff will comprise five Pathways Care Teams that work both in the field and in health care settings to serve this vulnerable population.

Children’s Hospital Colorado’s Fever in Infants 0 to 60 Days pathways provide guidance on the evaluation and management of infants ages 0 to 60 days old with a fever. We do this by working in partnership with individuals, their families and friends, the sector and community. Our aim is to maximise each client’s health and wellbeing, offering the help and support they need. Our focus is on supporting our clients, reducing substance-related harm and improving personal and public health.

Downtown Women’s Center was awarded $500,000 over three years to expand its model to support aging in place for older adult women residing in Permanent Supportive Housing . DWC’s work is influenced by the CAPABLE model, a program developed at the Johns Hopkins School of Nursing for low-income seniors to safely age in place. The grant will allow DWC to extend services to 245 additional units of PSH over three years and add RN and OT staff to serve aging women in these units.
Pathways To Health is a Long Island based holistic treatment and education center designed to provide physical and psycho-spiritual services, effectively integrating traditional and alternative forms of healing. Some healthcare professionals listed on our website have medical privileges to practice at Children’s Hospital Colorado, but they are community providers. They schedule and bill separately for their services, and are not employees of the Hospital. Children's Hospital Colorado partners with NRC Health to gather star ratings and reviews from patients, residents and family survey data. This provider either practices in a department or specialty that we currently do not survey, or does not have at least 10 ratings in the last 12 months.

Mentors the clinical orientation of a new clinical provider per department orientation process, as assigned by the Home Health Aide Program Director or Home Health Aide Manager. Attends educational in-service trainings, department meetings and other agency meetings as requested by the manager. Attends and actively participates in the hospice interdisciplinary group meetings.
